This information may be in verbal, written or electronic formats.
Collection of Personal Health Information
The CDFHT collects your health information directly from you or from the person acting on your behalf. In addition, the Chapleau and District Family Health Team may collect health information about you from other sources, after receiving your permission to do this when it’s needed.
Uses and Disclosures of Personal Health Information
The Chapleau and District Health Team uses and discloses your personal health information in a number of ways, including to:

The Chapleau and District Family Health Team discloses your personal health information, when requested by other health care service providers who request it in order to support your care and treatment.
You may tell us not to share your personal health information with certain people or organizations, subject to legal exceptions. Contact the Privacy Officer if you want to restrict us in disclosing your personal health information.
Access to Your Health Information
If you would like to access your health information, contact the office to arrange a time when a health care provider can review your entire record with you.
Correction
If you believe that the health information recorded about you by the Chapleau and Family Health Team is inaccurate or incomplete, please advise contact our Privacy Officer.
