The Chapleau and District Family Health Team (CDFHT) is committed to protecting the privacy of your information, by ensuring that effective policies and practices are followed.  These policies and practices comply with applicable laws and regulations and are based on industry best practices.  Through them, we:

 

• Take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal

• Conduct audits and complete investigations to monitor and manage our privacy compliance

 

In addition to safeguarding the privacy of your personal health information, the Chapleau and District Family Health Team’s policies and practices ensure that you can:

 

• Access your personal health information

• Request correction of incomplete or inaccurate information contained in your health record

• Have your personal health information kept confidential

• Request that your personal health information not be given to other health care providers

Personal Health Information

Your health information includes identifying information about

• Your contact information

• Your health card number

• Your substitute decision maker (if you have one)

• Records of your physical or mental health

• Records of treatment and care, and

• Other information included in your health record

 

This information may be in verbal, written or electronic formats.

Collection of Personal Health Information

The CDFHT collects your health information directly from you or from the person acting on your behalf.   In addition, the Chapleau and District Family Health Team may collect health information about you from other sources, after receiving your permission to do this when it’s needed.

 

Uses and Disclosures of Personal Health Information

The Chapleau and District Health Team uses and discloses your personal health information in a number of ways, including to:

• Treat and care for you

• Get payment for your treatment and care

• Plan, administer and manage the Chapleau and District Family Health Team and its programs

• Conduct risk management and quality improvement activities

• Conduct teaching and research activities

• Compile statistics

• Comply with legal and regulatory requirements and

• Fulfill other purposes as required by law

The Chapleau and District Family Health Team discloses your personal health information, when requested by other health care service providers who request it in order to support your care and treatment.

 

You may tell us not to share your personal health information with certain people or organizations, subject to legal exceptions.  Contact the Privacy Officer if you want to restrict us in disclosing your personal health information.

 

Access to Your Health Information

If you would like to access your health information, contact the office to arrange a time when a health care provider can review your entire record with you.

 

Correction

If you believe that the health information recorded about you by the Chapleau and Family Health Team is inaccurate or incomplete, please advise contact our Privacy Officer.
