Our Home Care RN and RPN provide services through the NE LHIN, which may include: 

• a home visit within 24 hours of discharge from the hospital

• medication review

• comprehensive nursing assessment

• ongoing wound care

• dispensing of IV antibiotics

• patient and family education

You can refer yourself, or a referral can be made by your doctor, nurse, community service provider or other health-care professional. A referral can also be made by a family member, a friend or a neighbour. A simple call gets the process started.
