Acute Episodic Care
Acute Episodic Care consists of procedures ordered by a physician or a nurse practitioner. If you have a physician, you can be referred to CDFHT to have these procedures completed. If you do not, you can call the clinic for more information.
About the Acute Episodic Care Program
A team of Nurse Practitioners conduct virtual visits over the phone for patients regarding NON URGENT issues, order and review test results, manage chronic conditions and prescription renewal
Locum physicians are also available for patients who require an in person physical assessment.
Nursing staff also see patients that have been referred by a physician. Treatments available through this program include:
