Discharge Program
The Discharge Program connects patients with the CDFHT to complete a wellness check after discharge from the hospital.
About the Discharge Program
Once a patient is discharged from the hospital, the CDFHT will call within 7 days of discharge to ensure follow up with an appropriate provider, even if you do not currently have a family physician.
