  • What is a Family Health Team?
    A Family Health Team is a primary health care model that brings together different health care providers to co-ordinate the highest possible quality of care.
  • Can anyone attend the Family Health Team?
    The Chapleau & District Family Health Team serves patients in the Chapleau & District, including Sultan and Foleyet. Anyone in our service area can access the programs and services at our location. If you are unsure about using one of our programs or services, call our office at (705) 864-0210, and our knowledgeable staff will help you navigate through our health care system
  • Is the Family Health Team part of the hospital?
    No, the Family Health Team is completey separate from the hospital. We are part of the Association of Family Health Teams of Ontario and have our own Board of Directors.
  • Are there physicians at the Family Health Team?
    Physician appointments can be accessed through the Family Health Team with our Locum (visiting) physicians. (see calendar for schedule) Appointments are also available with our team of Telemedicine Nurse Practitioners. We work very closely with the physicians serving our town and even though they are not located at our address, they oversee many of our collaborative programs and services and regularly host clinics at our facility. .
  • How do I sign up for one of the programs?
    Just call us and the reception will guide you through the process. Some programs appointments can be booked online. Look for the link in our programs section! Call our office at (705) 864-0210 for more information on specific programs and services. You do not need a family physician to access our programs. If you are unsure of what services or type of appointment you need just give us a call and one of our knowledgeable staff will help you.
