Can anyone attend the Family Health Team?

Some of our programs and services require a referral from your family physician. Only patients who do not have a family physician are able to be seen by our Nurse Practitioner. Many of our community engagement programs are available to all community members. If you are unsure about using one of our programs or services, call our office at (705) 864-0210.

Are there physicians at the Family Health Team?

Although there are no physicians at our location, we work closely with the physicians in Chapleau. Physicians are able to refer their patients to the Family Health Team if they could benefit from one of our programs or services.

Is the Family Health Team part of the hospital?

No, the Family Health Team is completey separate from the hospital. We are part of the Association of Family Health Teams of Ontario and have our own Board of Directors.

What is a Family Health Team?

A Family Health Team is a primary health care model that brings together different health care providers to co-ordinate the highest possible quality of care.

How do I sign up for one of the programs?

For some of the programs, such as CDFHT Presents and Active Chapleau it can be as simple as calling our office to register for an event. For other programs, you may need a referral from your family physician. Call our office at (705) 864-0210 for more information on specific programs and services.
